In-Home Care Assessment
FREE IN-HOME ASSESMENT: This is the first step to retain Kentucky Premier Personal Care LLC as your in-home personal care service agency. This enables Kentucky Premier Personal Care to develop a care plan.
We promote independence at home. We focus our care plan on YOU! This is how we make a plan to meet your needs. We look at what task you may need assistance with. We look at clients' strengths and weakness during this assessment. While completing the assessment it allows our care team to answer pricing questions and review any insurance policies the client may have that could pay for services.
This is also the time we look at the hours of care a client may need. We receive consent during this time to contact primary care providers to work with them to provide you with the best in-home care experience possible. We work with your circle of care team to meet your own personal care goals.
